This panel forms the right wing of the Isenheim Altar, a polyptych commissioned for the hospital chapel of the Monastery of St. Anthony in Isenheim. Matthias Grünewald created this work to provide comfort to patients suffering from ergotism, a condition often associated with hallucinations and skin afflictions. The imagery depicts the saint under assault by a collection of grotesque, hybrid creatures. These demons possess features of birds, reptiles, and mammals, reflecting the period's fascination with the monstrous and the supernatural.

Grünewald employs a palette of harsh, unnatural colours to heighten the sense of psychological distress. The saint appears on the ground, his posture suggesting both physical struggle and spiritual endurance. Above the chaos, a celestial light breaks through the dark, turbulent sky, offering a contrast to the terrestrial torment below. The composition is dense, with figures overlapping in a chaotic arrangement that denies traditional spatial depth. This approach forces the viewer to confront the visceral nature of the saint's ordeal.

Technically, the work demonstrates the artist's mastery of oil paint, using glazes to achieve luminosity in the figures and the surrounding landscape.
