The composition follows the iconographic tradition of the lactatio Bernardi, where the Virgin Mary offers milk to the saint, symbolising his spiritual nourishment and devotion. The artist employs a rigorous system of hatching to define the architectural space, which is set within a late Gothic interior.

Saint Bernard occupies the foreground, his figure rendered with heavy drapery folds that suggest the weight of his monastic habit. He holds a crozier, a symbol of his ecclesiastical rank, while his posture conveys humility and awe. Opposite him, the Virgin sits upon an elevated throne, cradling the Christ Child.
